The Arc of Yates is expanding services to provide more support for children and youth in Yates County. Children’s Health Home Services are Medicaid funded Care Management services that support children/youth under the age of 21 who have chronic health, medical or mental health conditions and their families.

George Roets, Yates County Director of Community Services, says,  “We are very pleased with the decision of The Arc of Yates to become a Children’s Health Home Care Management Agency. This will serve to strengthen our response to children of Yates County. This comes at a time when the community is coming together to develop a System of Care approach aimed at better meeting the needs of all children and families in Yates County.”

The Arc of Yates has partnered with Children’s Health Home of Upstate New York (CHUNNY) and has become a designated Care Management Agency. An assigned Care Manager will work with children/youth to assure that they receive everything necessary to stay healthy in the community, out of the emergency room, and out of the hospital.

Arc Executive Director Allen Connely stated, “We are excited to be able to provide a service that strengthens our impact and reaches outside the realm of our typical services. There is a huge need within the community for services like Health Homes and being a positive influence within our community is exactly what our mission is all about.”
